Real Ukrainian-language casework
The first case is a medical record; the second is a civil one. Together they show the two checks Ukrainian packets go through: honest handling of hard handwriting, and names transliterated from Ukrainian rather than Russian.

Ukraine · handwriting
We translated every legible clause, marked the rest as illegible line by line, and the packet moved forward on what the source could actually support.
Read the case
Ukraine · transliteration
We transliterated from the Ukrainian, not the Russian, so Oleksandr stayed Oleksandr and matched the passport the packet was built around.
Read the caseDocument-family evidence
Which record you can actually obtain depends on where the paper is and what ЕСОЗ holds. We name what you have — and what your doctor can still issue — before you pay.
The Карта профілактичних щеплень has been the clinic-kept vaccination card since the 2006 order that created it. Families rarely hold it themselves — which mattered little until so many families moved.
Soviet-era pages beneath it mix print and pen: Ukrainian forms, Russian-style shorthand like АКДС, and seals across the entries. Every layer is translated; nothing is guessed.
Left without papers? The довідка про проведені щеплення is generated from 063/о and ЕСОЗ by your family doctor — electronically signed — and we translate it with the signature explained.
Ukraine’s eHealth system holds the dose history clinics recorded into it. Health-ministry guidance for displaced families names it as a source the довідка draws from, alongside the paper forms.
A довідка arrives electronically signed or stamped on paper. We preserve whichever authentication it carries and describe it for the U.S. reviewer.
Hospital extracts — form 027/о — and the Історія розвитку дитини carry the clinical story. Wartime copies are often hurried scans and phone photos.
We translate what the image supports and mark the rest honestly, because a reviewer trusts a record that admits its gaps more than one that hides them.
How it works
Upload what you hold — or what your doctor sends. We identify each record, confirm the page count and the price, then translate, cross-check, and certify.
Send картки, довідки, виписки, both sides of loose pages, and any letter naming what the office wants. Hurried wartime scans are workable.
A Ukrainian specialist translates print and handwriting, both languages where they mix. A second reviewer checks names against passport transliteration, every date, and each seal.

Ask for fixes before we certify. You then receive the source copy, the complete English translation, and the signed and stamped Certificate of Translation Accuracy.

“We left without our daughter’s картка. Her doctor emailed a signed довідка, and the translation explained the electronic signature so the exam doctor accepted it.”

Questions before ordering
The rule holds: Ukrainian-language evidence in a U.S. filing needs a complete certified English translation. Which record carries your doses depends on what the war left reachable.
The price is $24.95 per physical page, with no word cap. A довідка runs one or two pages; cards and hospital files go longer. We confirm the count before you pay.
Most one- to three-page orders arrive within 24 hours. Old handwriting and long hospital files get a confirmed delivery window before we start.
Your package carries the source copy, a complete English translation, and a signed and stamped Certificate of Translation Accuracy. If USCIS raises a translation issue, we revise it free. The record underneath still has to be the right one.
Your family doctor in Ukraine can issue a довідка про проведені щеплення from form 063/о and the ЕСОЗ record — electronically signed, or on paper with a stamp. We translate it with the signature block explained.
It is a legitimately issued record, and we translate it with its electronic signature preserved and described. A translator’s note tells the reviewer how the document authenticates — the same approach we use for other e-issued records.
It is the Soviet-era shorthand for the DTP series — diphtheria, pertussis, tetanus. We land it on the names a U.S. worksheet uses and keep the source shorthand in brackets.
USCIS generally asks for a certified English translation, not a notarized one and not an apostille. A school, insurer, or foreign authority may set its own rules, so follow that recipient’s checklist.
It is normal for the region and not a problem. Both language layers are translated within one certified file, and the certificate names the languages involved.
It documents the COVID doses it lists, and we translate it when needed. Childhood series still come from the картка, the довідка, or the hospital archive — send those alongside it.
The passport’s. We transliterate names from the Ukrainian and cross-check every record in your packet against the passport spelling, flagging mismatches instead of hiding them.
What we check before certification
These issues repeat across wartime packets. The translation should carry the doses, the signatures, and the languages exactly, with nothing left to decode.
Our check: The довідка authenticates through its signature block. We preserve it, translate its labels, and explain the format in a note.
Our check: Soviet abbreviations are landed on recognizable vaccine names with the source kept in brackets, so the worksheet cross-walk is instant.
Our check: Ukrainian print with Russian-style penwork is translated as two layers of one document, never half-done.
Our check: We flag cut-off edges and faded lines before work begins, so the packet is built on what the source actually shows.
The standard behind these checks
AcceptReady™ is CertTranslate's published standard for certified translations: complete word-for-word translation, mirrored page layout, signed and stamped Certificate of Accuracy on letterhead, and matched to the receiver's published requirements.
How to check each one yourselfChecked before we certify. If a reviewer later asks for a translation-related correction, the revision is free.

The immigration medical exam leads: the doctor credits doses from written, dated records, and bringing them in English is the applicant’s side of the process.
File the Ukrainian record together with its certified English translation. The translation must never replace the source, shorten it, or quietly tidy it.
Картки and довідки prove the childhood series. Dated, translated rows keep doses from being repeated at the exam.
Medical evidence filed with USCIS needs a complete certified English translation of every Ukrainian-language page.
Health offices read the translated dose table against their checklist — довідки from ЕСОЗ included.
A виписка becomes the treatment history a U.S. doctor plans from — its dates and terms must survive exactly.
Document glossary
These terms travel together on Ukrainian records, but they name different documents, systems, or parts of the history.
