The personal certificate and the clinic card carry the same doses with the same standing. Emigrants often hold neither, and the fix is a выписка the clinic issues from form 063/у — relatives frequently obtain it.
Moscow shows vaccinations in the city’s electronic medical record, and a federal online certificate began piloting on Gosuslugi in 2026. Records older than the systems still live on paper.
Request the выписка before your I-693 appointment. The doctor credits written, dated doses — and a fully dated extract keeps childhood series from being repeated.
Kazakhstan
The 063/у card that opens school doors
Kazakhstan kept the Soviet form family: the карта 063/у and the прививочный паспорт document childhood doses, and the card is required for kindergarten and school admission.

Belarus
