Polish Death Certificate Translation

**Przyczyna zgonu (cause of death):** The cause of death recorded on a Polish death certificate comes from the Karta Zgonu (medical death card) issued by the attending physician or coroner. The Karta Zgonu follows WHO International Classification of Diseases (ICD) formatting: przyczyna wyjściowa (underlying cause), przyczyna wtórna (secondary cause), and przyczyna bezpośrednia (immediate cause).
