A divorce agreed at the civil registry produces a свідоцтво про розірвання шлюбу. This is the short certificate most filers hold.
It names both former spouses, the registry office, the act record number, and the date. Where a surname went back to its earlier form, that shows here too.

Рішення суду — and no certificate follows
For court divorces granted on or after 27 July 2010, Ukraine stopped issuing a свідоцтво. The court decision itself is the proof, once it has entered legal force.
U.S. visa reciprocity guidance accepts that court decision in place of a divorce certificate for divorces from that date. A missing свідоцтво is not a missing document.

A court divorce granted before 27 July 2010 was registered afterwards, and a свідоцтво was issued. For those years, expect both the decision and the certificate to exist.

Yes. Records from the Ukrainian SSR are often bilingual, with printed labels in Russian and Ukrainian and the entries handwritten in one of them. We translate the whole form and say which language each part was in.
